Scale SO1 Salary £26,470 – £28,221

We are seeking a dynamic and motivated individual to provide specialist clinical expertise and support using an integrated approach to the identification, assessment and support of carer’s health and wellbeing needs across health and social care in order to maintain the capacity of the carer to care. You will also ensure that carers are identified and supported using a whole family approach across all age groups and categories of care.

You will be able to bring to this role your experience of providing specialist clinical assessment, evidence based intervention, practical advice and emotional support for family carers at all stages including bereavement. Supporting the independence, physical health and emotional wellbeing of carers and their families, you will enable carers to manage their caring roles and to have a life outside of caring, ensuring carers receive the right support at the right time, in the right place.

In this role you will work closely with Primary Health Care teams, specialist services and multi-agency partners, participating in and facilitating developments within the community service to meet the health and social care needs of the population. Excellent communications skills will therefore be required, with the ability to use these effectively with a range of audiences.
