Housing Related Support Officer  

Fixed Term Contract until 31st March 2019.

£21,693 to £24,657 pro rata

37 Hours per week

 

Working across the UHL Hospitals and Bradgate Mental Health Unit, your role will be to provide housing related support to patients being discharged from hospital, promote holistic recovery and enable independent living and social inclusion.

To provide case management and organise support services for individuals and families to prevent homelessness, increase self-reliance and build capacity to live independently. To support service users/patients to secure and move into independent accommodation, assisting with the practicalities of moving.

To encourage and support tenants to live as fully and independently as possible within the local community, providing information, emotional, organisational and practical support and training as appropriate.

You will have previous experience of working within a support or care setting dealing with vulnerable people with a range of support needs, undertaking assessments, support planning, formulating action plans and reporting outcomes.

 

You will also have an understanding of issues facing vulnerable people (in particular those with mental health issues) with housing related support needs including dependency needs, homelessness and general life skills.

Please note that this job requires the post holder to visit different wards which may be some distance from each other and attend meetings at various different locations in the hospitals.

This is a fixed term contract post until the 31st March 2019.
