We are seeking an enthusiastic, energetic and motivated Level 3 social worker to join our dynamic and supportive multidisciplinary team working across Health Transfers with a range of service users. You will support complex discharges that require your specialist social work skills to facilitate safe and timely discharges from the hospital setting back to the community.
Your role will be key to ensuring that the hospital team can respond to increasing demands with the continued delivery of a high quality service promoting choice and independence. You will ensure the timely completion of quality assessments and reviews of discharged patients.
Requirements of the post:
- Social Work BA or MA, CQSW;
- HCPC Registration;
- Good written and verbal communication skills;
- Ability to work in partnership with people and their carers;
- Experience of working in a multi-cultural area;
- Commitment to anti-discriminatory practice;
- Good time management skills;
- There will be some evening and weekend work by the post holders;
- This post is subject to an Enhanced DBS check.
