The Housing Enablement Team (HET) is a dynamic and flexible service designed to support patients in hospital who have housing issues that are preventing their discharge. HET was established in 2014 and has since expanded across Leicester, Leicestershire & Rutland. You will assist the service by providing advice and support to patients around housing, homelessness, and tenancy related issues within both physical health and mental health settings.
You will assist with a variety of housing related referrals from medical staff in relation to patients who need support to aid discharge.
You will work in a multi-disciplinary team setting as a housing professional and ensure effective liaison with key partners and officers in associated organisations and Councils to develop cross boundary strategies on promoting and delivering against the reduction of delays to hospital discharge.
You will have previous housing/homelessness experience as well as strong knowledge of preventative homelessness practice, private rented sector housing and housing legislation. Basic working knowledge of the social care and health sectors will also be useful but not essential.
You will be able to work individually and as part of a team with minimum supervision, managing your own caseload and be able to assess a patient’s housing needs and plan and carry out resolutions to secure accommodation to enable discharge.
You will be able to deal sensitively with customers/patients with a variety of support needs, including mental ill health and be able to support patients in potentially difficult situations.
You will be based at the Bradgate Mental Health Unit and Leicester Royal Infirmary on a rota basis and also work with patients in the Mental Health Rehab Sites at the Willows and Stewart House.
