Social Care Practitioner

We are looking to recruit to a permanent Social Care Practitioner post within the Short term assessment and reablement team in South Derbyshire.

We welcome applicants with experience of working in an Adult Social Work setting. To demonstrate an ability to work with a person centred and strengths based approach to achieve the ideal outcomes with people to live as independent as possible within their own homes and communities is essential. Experience of a collaborative approach to working with partners in statutory and voluntary organisations is required to support people to return home or remain in their own homes following a period of ill health or change in needs. 

The successful candidate will need to have knowledge of the current legislation that underpins good practice, including the Care Act 2014 and Mental Capacity Act 2005, as you will be working with people to assess their care and support needs to enable them to live as independently as possible in their own homes.
