Derbyshire Dales START (Short Term Assessment and Reablement Team)

An exciting opportunity has arisen for a Social Care Practitioner to join Derbyshire Dales Short Term Assessment and Reablement Team (START) working predominantly across Derbyshire Dales South.

START provides short‑term, strengths‑based interventions to support adults to regain independence following illness, crisis, hospital discharge or a significant change in circumstances. The service focuses on assessment, reablement and prevention, ensuring people receive the right support at the right time and avoid unnecessary long‑term care.

As a Social Care Practitioner, you will work as part of a multi‑disciplinary team, reporting to the Senior Practitioner and Service Manager. You will undertake Care Act compliant assessments, support planning and reviews, working closely with people, their carers, health colleagues and reablement providers to promote independence and positive outcomes.

The people you work with may have physical health needs, mental health needs, learning disabilities, dementia, frailty or other complex needs. You will take a holistic view of people’s circumstances, identifying strengths, networks and community resources alongside eligible needs.

You will play a key role in:

  • Completing short‑term assessments to inform reablement and ongoing support
  • Applying a strengths‑based, person‑centred approach, with a strong emphasis on co‑production
  • Supporting safe and timely hospital discharges and preventing admissions
  • Working collaboratively with health, reablement services and wider partners
  • Ensuring the person’s voice, choice and independence remain central to all decision‑making

You will work closely with people who access our services and their carers, actively seeking and valuing lived experience to continuously improve practice.
