An exciting opportunity has arisen to join Derbyshire County Council’s Public Health Department, within Live Life Better Derbyshire, to support the work of the Health Improvement Team.
Live Life Better Derbyshire (LLBD) supports people to identify their holistic health and wellbeing needs, and provides weight management, smoking cessation, and physical activity services across Derbyshire. Additionally, LLBD also provide Health Check, Hypertension, National Child Measurement and Healthier Futures services.
We have a wonderful opportunity to recruit a new Health Improvement Worker to join the LLBD Team Hub Team.
The LLBD Hub is the first point of contact for all referrals into the service, as well as general enquiries from clients, referrers, partners, and staff.
Health Improvement Workers supporting the hub will contribute to the delivery of the LLBD service by supporting clients over the phone to identify and address their health and wellbeing needs, and by assisting with the various referral processing pathways within the hub team.
In addition, the post holder will also support with a range of other duties to ensure that clients are fully supported in a timely and effective manner. Responsibilities may include (not exhaustive):
This post will be based within the LLBD Hub Team located in Matlock. Working within a busy office, the successful candidate will require a good level of IT literacy including proficient use of Microsoft Teams, Word, Excel, and Outlook along with the ability to adapt to bespoke databases. The ability to use Microsoft Office 365 and its associated applications would be an advantage. It is also essential that the successful candidate has excellent communication skills with the ability to communicate with a wide range of clients being aware of barriers that may prevent a lifestyle change. Due to the nature of the service the successful candidate must demonstrate the ability and commitment to undertaking a comprehensive range of duties as well as demonstrating flexibility in order to meet the demands of the service. There may at times be a need to work out in the Derbyshire community.
